We are currently seeking a passionate and experienced Home Share Coordinator to join our Outreach team. This leadership role is ideal for someone who is highly organized, relationship-focused, and committed to supporting inclusive and person-centered services within the community. About the Role Reporting to the Senior Program Manager – Outreach, the Home Share Coordinator is responsible for overseeing the administration and coordination of Hollyburn’s Home Share and Community Inclusion programs. This position supports Home Share providers, Community Inclusion Workers, clients, and families while ensuring services are delivered in accordance with CLBC and agency standards. The Home Share Coordinator plays a key role in program development, contract management, case coordination, quality assurance, and stakeholder collaboration throughout the Lower Mainland.

Requirements

  • Bachelor’s degree in Social Services or an equivalent combination of education and experience
  • Minimum 5 years of experience in social services
  • Minimum 1 year of supervisory experience
  • Strong understanding of case management, contracts, and program governance
  • Excellent communication, organizational, and relationship-building skills
  • Ability to effectively prioritize and manage multiple responsibilities
  • Strong critical thinking and decision-making abilities
  • Experience working collaboratively with families, contractors, government agencies, and stakeholders
  • Strong administrative and documentation skills
  • Proficiency with Microsoft Office Suite including Word and Excel
  • Ability to work independently and adapt in a fast-paced environment
  • Ability to travel frequently throughout the Lower Mainland
  • MANDT Training (Training available to those who require this)
  • First Aid & CPR Certification (Training available to those who require this)
  • Must have access to a vehicle, hold a valid B.C. Class 5 driver’s license, and have business insurance with $3,000,000.00 liability
  • Must be able to pass a Criminal Record Check

  • Experience within Home Share or Community Inclusion programs

Responsibilities

  • Coordinate and oversee Home Share and Community Inclusion services
  • Recruit, screen, approve, and support Home Share providers
  • Support program development and implementation based on CLBC requirements
  • Develop strong communication pathways with clients, families, providers, and community stakeholders
  • Monitor program quality standards and track service outcomes
  • Participate in contract planning and negotiations with government agencies
  • Address complex service delivery and cross-program issues through collaborative problem-solving
  • Coordinate training and ongoing support for Home Share providers
  • Facilitate meetings with healthcare professionals, families, clients, and providers
  • Conduct client and provider visits, environmental assessments, and service evaluations
  • Identify and manage risk issues while implementing appropriate corrective actions
  • Monitor client satisfaction, progress, and continuity of services
  • Maintain an active caseload of clients and Home Share providers
  • Provide supervision and support to Community Inclusion Workers and Home Share providers
  • Foster a positive, collaborative, and accountable team culture
  • Approve staff time entries and complete program administrative duties
  • Ensure leadership is informed of program developments and concerns
  • Participate in strategic planning, team development, and continuous improvement initiatives
  • Stay informed on industry trends, legislation, and best practices
