About The Position

This role will provide onsite support as the Enhanced Service offerings of Care Coordination and Home Services begin at Concord Reserve. This individual will be an LPN who will play an integral part of campus associate relationship building as resident needs are identified. This position will be responsible for maintaining contact with residents who are receiving care coordination as well as assisting to educate residents on Care Coordination and Home Services as appropriate. This individual will work as a team member and use their specialized credentials and/or expertise to serve clients. This position requires excellent communication skills with team members, clients and family members and an elevated level of patience and compassion for others.

Requirements

• High School Diploma or equivalent preferred
• Previous leadership experience
• Previous experience in a Home Services position, or equivalent
• Good computer skills, including use of Microsoft Office and data entry
• Must possess a customer service background and orientation
• Valid Ohio driver's license required

Responsibilities

• Coordinates services provided by Home Services Assistants to meet client's personalized needs and educates residents on the services available
• Performs services to meet client's personalized needs as required
• Supports Home Services Assistants to maintain program policies and procedures
• Provide administrative support, including (but not limited to): Obtain resident agreements when necessary, Scheduling and coordinating home services with clients and associates (including after normal business hours), Answering phone calls, including emergency (including after normal business hours), Performing data entry into software systems, Performing monthly billing, completing and submitting insurance paperwork, Generating necessary reports, Ordering and maintaining supplies, Filing medical data and other pertinent information, Maintaining records for Home Services
