This role will provide onsite support as the Enhanced Service offerings of Care Coordination and Home Services begin at Concord Reserve. This individual will be an LPN who will play an integral part of campus associate relationship building as resident needs are identified. This position will be responsible for maintaining contact with residents who are receiving care coordination as well as assisting to educate residents on Care Coordination and Home Services as appropriate. This individual will work as a team member and use their specialized credentials and/or expertise to serve clients. This position requires excellent communication skills with team members, clients and family members and an elevated level of patience and compassion for others.
