About The Position

Assist the pharmacy manager in the operation and management of the pharmacy, provide clinical care to pharmacy patients, provide indirect supervision for support staff, and ensure adherence to policies and procedures. The position is based at Seven Lakes Pharmacy, 3650 Victoria St N, Shoreview, Minnesota. The schedule is 1.0 FTE (80 hours per two weeks), with hours ranging from 7:30 a.m. to 5:30 p.m., rotating weekends, and rotating on-call duties.

Requirements

B.S./B.A. or Pharm.D. in Pharmacy from an accredited school of pharmacy.
One year of experience as a practicing pharmacist.
Minnesota pharmacy license.
High school diploma or GED is listed as a qualification.

Responsibilities

Perform initial patient evaluations related to prescribed therapies; collect, analyze, interpret, and evaluate relevant medication-therapy data.
Obtain patient histories and use them with other patient data to optimize drug therapy management.
Work with other healthcare providers to evaluate patients for the appropriateness of home IV therapy.
Evaluate signs and symptoms in relation to disease processes and recommend modifications to drug therapy.
Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy.
Monitor patient records for potential drug-drug or drug-food interactions and patient allergies.
Counsel patients, families, and healthcare professionals and answer questions concerning drug therapy.
Provide prescription drug therapy, ensuring accurate labeling and timely dispensing of medications.
Become familiar with home care and pharmacy billing processes.
Evaluate patients’ supplies of medications and IV access supplies.
Assist patients and other healthcare providers with maintaining IV lines for medication administration.
Monitor and evaluate clinical outcomes.
Ensure prescriptions are dispensed in accordance with legal requirements, regulatory agency requirements, and Fairview Home Infusion policies and procedures.
Maintain proficiency in pharmacy practice skills and theory by reviewing literature and attending in-services, seminars, and conferences.
