About The Position

Amedisys is hiring a full-time Home Health Registered Nurse for the Middle Westchester County territory. The position pays $115,000 to $120,000 and offers a $20,000 sign-on bonus. The RN performs patient assessments and works with the care team to develop and implement plans of care, promoting patients’ health and independence through teaching and rehabilitative measures. The role also includes supervising LPNs and HHAs, documenting care, communicating patient progress, providing staff education, and completing assigned on-call duties. Requirements

• At least one year of clinical experience as a Registered Nurse. Candidates with less than one year must be approved by the VP Clinical.
• Current RN license specific to the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of clinical RN experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines based on the patient's identified needs, or document the rationale for not making referrals.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
