About The Position

The Home Health RN assesses patient conditions, implements care plans, and provides ongoing education and support to patients and their families. Care is delivered in accordance with the organization’s philosophy, policies, and community health practice standards. The role also performs admission processes and case management responsibilities in accordance with MGB Home Care policies and standards of practice, using thorough and timely electronic documentation and interdisciplinary communication. The position requires travel to assigned cases and participation in weekend rotations once every six weeks.

Requirements

• At least one year of Certified Home Care experience specifically within a certified home health agency.
• At least one year of case management experience.
• Ability to travel to assigned cases.
• Experience in hospice, private care, or working as a Case Manager outside a certified home health agency does not satisfy the certified home health care management experience requirement.

Responsibilities
