About The Position

Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years. Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services. Maintain thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and MGH Home Care standards of practice. This is a float position serving Marathon; travel to assigned cases is required, and weekend rotations occur once every six weeks.

Requirements

• Must have a Physical Therapist professional license in Massachusetts.
• At least one year of home care experience is required.
• Ability to travel to assigned cases.
