About The Position

The On-call RN is responsible for handling calls from referral sources and patient care-related calls, including any necessary visits and administrative tasks. This role operates on a 7-day on, 7-day off schedule. The work includes weekday evening call from 4:30 PM to 8:00 AM the next morning, and weekend call from Friday 4:30 PM to Monday 8:00 AM. The position may also cover holiday hours during the assigned week. While not visiting patients or performing weekend hospital list/transfer OASIS duties, the On-call RN has the flexibility to work from home. Key responsibilities include determining patient eligibility for care, completing OASIS assessments, and developing individualized care plans in accordance with professional standards, agency policies, and federal and state laws. The On Call RN plays a crucial role in achieving optimal quality and process outcomes.

Requirements

  • Registered Nurse with a current Alabama license.
  • Two years of registered nurse clinical experience required.
  • At least 6 months of home health or hospice experience.
  • Excellent coordination and communication skills.
  • Detail-oriented and able to work with minimal supervision.
  • Must be able to read, write legibly, speak, and comprehend English.
  • Must have current BLS certification or be able to obtain within 60 days of employment and before first independent patient visit.
  • Must be licensed and insured to operate a motor vehicle.

  • Extensive clinical knowledge and experience in the care of patients with multiple and complex diagnoses, disease processes and care needs.

Responsibilities

  • Respond to all answering service calls.
  • Communicate with patients and families.
  • Establish positive working relationships with physicians and their staff, keeping them informed of patient progress.
  • Assist patients and families (via phone or visit) to prevent condition exacerbation or avoidable ER visits/hospitalizations.
  • Provide Clinical Managers with summaries of all activity, interventions, and recommendations.
  • Perform assigned chart audit activities.
  • Provide skilled nursing care.
  • Revise the plan of care based on identified patient needs and progress.
  • Review and update the medication list.
  • Communicate patient care activities with the RN Care Manager.
  • Assess patient outcomes and suggest improvements.
  • Complete all required documentation in the EMR.
  • Check the DCH hospital list on weekends.
  • Complete a Transfer OASIS for patients admitted and remaining in the hospital for 24 hours.
  • Communicate all findings with the Administrative RN/Manager on-call.
  • Provide the Clinical Manager with a summary each morning by 0800.
  • Complete visits of any type assigned.
  • Perform an initial assessment of patient and family to determine home care needs.
  • Provide a complete physical assessment and history of current and previous illnesses.
  • Explain patient’s rights and expectations, HIPAA, advance directives, and inform the patient and family of expected disciplines and frequency at the initial assessment.
  • Develop a plan of care corresponding with physician orders, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions.
  • Include the patient and family in the planning process.
  • Make revisions to the plan of care as patient status and needs change.
  • Communicate and update the primary physician when necessary.
  • Re-assess patients within 24-48 hours following a change in condition or discharge from an inpatient setting and update the plan of care.
  • Provide supervision of LPNs and home health aides per regulatory requirements.
  • Support and promote Agency policies and practices.
  • Maintain communication with team members.
  • Maintain continuing knowledge of competencies related to the Nursing profession.
  • Attend mandatory Agency meetings and in-services.
  • Participate in Quality Improvement and Corporate Compliance activities as assigned.
  • Perform essential job functions in a manner that ensures the safety of patients, visitors, and employees.
  • Identify and reduce unsafe practices that may result in harm to patients, visitors, and employees.
  • Recognize and take appropriate action to reduce risks and hazards to promote safety for patients, visitors, and employees.
