Registered Nurse (RN) / Licensed Practical Nurse (LPN) Location: Monument Valley, AZ Position Type: 1099 Independent Contractor Schedule: 10 hours per week Sign-On Bonus: $2,500 , paid in two installments — $1,250 at 90 days and $1,250 at 180 days. Giving Home Health Care is seeking a compassionate, skilled, and dependable Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide one-on-one nursing care to a patient in the comfort of their home. The nurse will provide care in accordance with the physician-established plan of care, while serving as an advocate for the patient and helping ensure their health, safety, comfort, and quality of life. Build Meaningful, Long-Term Relationships Once a patient is accepted into the program, there is no expiration date for this benefit, providing an opportunity for caregivers to build meaningful, lasting relationships with their patients over the years. In this role, you will work closely with the patient and collaborate with CNAs, HHAs, Case Managers, physicians, and other members of the care team to ensure the patient receives coordinated, compassionate care.
Requirements
- Current RN or LPN license applicable to the state of Arizona.
- Previous home health or in-home nursing experience preferred.
- Compassionate, patient-centered approach to care.
- Strong communication and clinical skills.
- Ability to work independently while collaborating effectively with the broader care team.
- Reliable and committed to providing consistent, quality patient care.
- Current CPR certification
- TB test and physical examination.
- Maintain required insurance.
- Good interpersonal skills.
- Hand-eye coordination and manual dexterity.
- Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients.
- Ability to operate under stressful conditions and make quick decisions.
- Must have dependable transportation and be willing to commute to the patient’s home.
- Ability to provide basic medical management up to medically fragile end of life care.
- graduate of an accredited college or university, Associate or Baccalaureate School of Nursing
- Current State or Compact License as a Registered Nurse or Licensed Practical Nurse.

- 1+ years of experience as an RN/LPN in a clinical or home health setting.

Responsibilities
- Provide skilled nursing care in the patient’s home in accordance with the physician-established plan of care.
- Develop a trusting, professional relationship with the patient and serve as their advocate.
- Monitor the patient’s condition and communicate changes to the appropriate members of the care team.
- Administer medications and treatments as ordered and within the nurse’s scope of practice.
- Coordinate and collaborate with CNAs, HHAs, Case Managers, physicians, and other care providers.
- Provide education and support to the patient and family as appropriate.
- Maintain accurate and timely clinical documentation.
- Promote patient safety, dignity, comfort, and independence.
- Follow all applicable professional, regulatory, and agency requirements.
- Execute physician prescribed plans of care.
- Complete compliant documentation of the care provided in the system of record.
- Manage and administer prescribed medication, treatment, and therapies.
- Conduct patient assessments, coordination of care.
- Perform various activities associated with daily living.
- Monitor vitals and GI intake and output.
- Assess skin integrity and administer wound care.
- Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression.
- Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients.
- Provide education and training to the patient, their family, and/or the Home Health Aide.
- Work with patients to be done one-on-one in the patients home to provide for a comfortable environment.
- Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants.
- Perform other personal care services as necessary to meet the patient’s needs.
