Requirements

Current RN License
Minimum 2 years of Home Health management experience
Current CPR Certification
Valid Driver’s License & Auto Insurance
Knowledge in HOMECARE HOMEBASE (HCHB) required.

Nice To Haves

Strong Home Health leadership experience
Experience managing clinical teams in a Medicare-certified agency
Knowledge of OASIS, compliance, and regulatory standards
Experience with QAPI and performance improvement initiatives
Excellent communication and clinician coaching skills
Ability to lead with accountability, compassion, and professionalism
HomeCare HomeBase experience preferred

Responsibilities

Leading day-to-day clinical operations for the home health team
Ensuring compliance with Medicare Conditions of Participation and regulatory standards
Managing and supporting nurses, therapists, and clinical staff
Driving quality initiatives, QAPI performance, and patient outcomes
Reviewing clinical documentation and ensuring Plan of Care compliance
Coaching and mentoring clinicians to achieve excellence in patient care
Supporting census growth while maintaining high clinical standards
Partnering with intake, operations, and interdisciplinary teams to ensure seamless care coordination
Identifying process improvements and implementing corrective action plans when needed
