Home Health RN Case Manager
About The Position
The Registered Nurse (RN) Case Manager delivers safe patient care using the nursing process of assessment, planning, implementation, and evaluation. The role provides direct patient care within the scope of practice and directs and guides patient and family teaching and the activities of other nursing personnel while maintaining standards of care. The RN Case Manager is directly responsible and accountable for care provided to assigned patients. The role communicates with physicians about changes in patients’ clinical conditions, including cardiac monitoring, diagnostic study results, and symptoms, and responds promptly and accurately to changes in condition or response to treatment. Requirements
- At least five years of work experience after graduation from an accredited school of nursing.
- Current state Registered Nurse license.
- Experience and knowledge in basic to intermediate computer skills.
Nice To Haves
- Previous Home Health experience.
- Current BCLS certificate.
- Five years of acute care nursing experience.
- CCM certification or ability to obtain it within one year.
- Knowledge of Milliman Criteria and InterQual Criteria.
