Requirements

An associate's or Bachelor’s degree with one year of home health care experience preferred.
AZ Nursing licensure in good standing.
Current CPR/BLS certification.
Fingerprint clearance card.
TB test results within the last year.
AZ Driver's license and proof of insurance with reliable transportation.
Excellent observation, verbal and written communication, and problem-solving skills.

Responsibilities

Completes an initial assessment of the patient and family to determine home care needs, including a complete physical assessment and history of current and previous illnesses.
Develops a care plan involving the patient and the family to establish goals based on nursing diagnosis, incorporating therapeutic, preventative, and rehabilitative nursing actions.
Provides direct patient care as defined in the State Nurse Practice Act.
Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.
Contributes and participates in care coordination and patient-centered care as a part of an interdisciplinary clinical care team of nurses, therapists, CNAs, and MSWs.
