About The Position
This Home Health RN Case Manager role in Oklahoma City offers the opportunity to apply clinical judgment and shape patient care plans within a full care team. The position is with Interim HealthCare of Oklahoma City, a locally owned, Veteran and nurse-owned agency established in 1999, known for its community involvement and high CMS rating. This role is designed for experienced nurses seeking more autonomy and a greater voice in patient care, moving away from unsustainable caseloads often found in bedside nursing.
Requirements
- Active RN license in Oklahoma
- CPR certification
- Strong assessment and critical thinking skills
- Experience in home health is ideal, but experienced nurses from any clinical background ready for a change are encouraged to apply.
Responsibilities
- Play a hands-on role in developing and adjusting the plan of care for patients, working alongside LPNs, therapists, and physicians.
- Complete visits, OASIS assessments, and documentation with a focus on the overall patient picture.
- Coordinate with the care team to ensure seamless patient care.
- Educate patients and families on diagnosis, medication, and home safety.
- Advocate for patients and drive positive outcomes.
