About The Position
Looking for a nursing role where you can use your clinical judgment, build relationships, and manage your day? Join Elara Caring as a Home Health RN Case Manager serving patients throughout Boston Proper, including Chinatown. You’ll provide skilled care in patients’ homes and help them recover, manage their health, and stay independent. And because parking is part of working in Boston, we provide a paid parking pass for this territory.
Requirements
- Current, unrestricted Massachusetts RN license
- At least one year of clinical nursing experience
- Strong assessment, communication, and time management skills
- Ability to work independently and travel throughout Boston Proper, including Chinatown
- Valid driver’s license, current auto insurance, and reliable transportation
Responsibilities
- Provide skilled nursing visits
- Develop and update care plans
- Manage an assigned caseload
- Coordinate with physicians and other clinicians
- Teach patients and caregivers how to manage medications, treatments, and health conditions at home
