About The Position
Brightstar Care is seeking an experienced and compassionate Skilled Registered Nurse (RN) to join our team. We are a home health agency with a deep commitment to quality, we are offering positions for Nurses seeking more flexibility, autonomy, are driven by a growth mindset, and who put patient care first. This role involves making skilled visits to service San Antonio and surrounding areas. The agency offers paid travel time and mileage reimbursement. Specific pay rates are provided for Medicare SOCs/Recerts ($115.00), Non-Medicare SOCs/Recerts ($90.00), and Treatments/DC ($50.00). A $5,000 retention bonus is offered after one year of employment for full-time nurses. The agency is specifically looking for nurses experienced with TruCare and regular visits.
Requirements
- Current and valid Registered Nurse (RN) license in the state of Texas
- CPR & BLS certification.
- Strong clinical skills and knowledge of home health care practices.
- Ability to work independently and manage time efficiently.

Responsibilities
- Conduct comprehensive assessments of patients' medical conditions and needs.
- Develop and implement individualized care plans in collaboration with the team.
- Administer medications, treatments and procedures as prescribed by physicians.
- Monitor patients' responses to treatments and make necessary adjustments to care plans.
- Maintain accurate and timely documentation of all patient interactions, treatments, and progress.
- Communicate effectively with patients, families, and other healthcare providers to ensure coordinated and optimal patient care.
