About The Position
Brightstar Care is seeking an experienced and compassionate Skilled Registered Nurse (RN) to join our team. We are a home health agency with a deep commitment to quality, we are offering positions for Nurses seeking more flexibility, autonomy, are driven by a growth mindset, and who put patient care first. This role involves performing skilled visits to service San Antonio and surrounding areas.
Requirements
- Current and valid Registered Nurse (RN) license in the state of Texas
- CPR & BLS certification.
- Strong clinical skills and knowledge of home health care practices.
- Ability to work independently and manage time efficiently.

Responsibilities
- Conduct comprehensive assessments of patients' medical conditions and needs.
- Develop and implement individualized care plans in collaboration with the team.
- Administer medications, treatments and procedures as prescribed by physicians.
- Monitor patients' responses to treatments and make necessary adjustments to care plans.
- Maintain accurate and timely documentation of all patient interactions, treatments, and progress.
- Communicate effectively with patients, families, and other healthcare providers to ensure coordinated and optimal patient care.
