Home Health Registered Nurse
About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, you will provide personalized, one-on-one care to help patients regain independence in their homes. You will work with physicians and clinicians to manage care plans that support recovery. The territory covered is West Orange County. The role includes providing skilled nursing care in patients’ homes, developing and implementing individualized care plans, monitoring patient conditions, educating patients and families, maintaining accurate care records, coordinating with healthcare professionals, reporting progress to physicians and the Clinical Manager, and overseeing Home Health Aides’ work and documentation. This is a patient-facing role subject to Humana’s TB screening program and driver safety requirements.
Requirements
- Diploma, Associate degree, or Bachelor’s degree in Nursing.
- Medical-surgical, ICU, ER, or acute-care experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities
- Provide skilled nursing care in patients’ homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and their families on disease management, medication, and treatment options.
- Maintain accurate records of patient care and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient’s physician and Clinical Manager.
- Oversee Home Health Aides’ work and monitor appropriate completion of documentation.
