Home Health Registered Nurse
About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, you will provide personalized, one-on-one care to help patients regain independence in their homes. You will work with physicians and other clinicians to manage care plans that support recovery. The role includes delivering skilled nursing care in patients’ homes, coordinating care with healthcare professionals, and overseeing Home Health Aides. This is a patient-facing position requiring TB screening and participation in Humana’s driver safety program, including maintaining a valid driver’s license and qualifying personal vehicle liability insurance. CenterWell Home Health provides personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. Its care teams include nurses, therapists, home health aides, and medical social workers who work together to support rehabilitation, recovery, and independence.
Requirements
- Experience in med-surg, ICU, ER, or acute care.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.
- One year of nursing experience.
- Home Health experience.
Responsibilities
- Provide skilled nursing care in patients’ homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and their families about disease management, medication, and treatment options.
- Maintain accurate patient-care records and coordinate with other healthcare professionals.
- Report patient-care and condition progress to the patient’s physician and the Clinical Manager.
- Oversee Home Health Aides and monitor their completion of appropriate documentation.
