Home Health Registered Nurse-Per Diem
About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence at home. You work with physicians and other clinicians to manage care plans that support recovery. The role involves providing skilled nursing care in patients’ homes, including medication administration, wound care, and patient assessments. You develop and implement individualized care plans, monitor patients’ conditions, educate patients and families, maintain accurate care records, coordinate with healthcare professionals, report progress to physicians and the Clinical Manager, and oversee Home Health Aides’ work and documentation. The position is per diem and patient-facing, with TB screening required if selected. It also participates in Humana’s driver safety program and requires a valid state driver’s license and personal vehicle liability insurance.
Requirements
- Med-surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities
- Provide skilled nursing care in patients’ homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and their families on disease management, medication, and treatment options.
- Maintain accurate records of patient care and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient’s physician and Clinical Manager.
- Oversee Home Health Aides and monitor appropriate completion of their documentation.
