About The Position
As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care to help patients regain independence in their homes. You will work with physicians and other clinicians to manage care plans that support recovery. This is a per-diem, patient-facing role involving in-home skilled nursing care, including medication administration, wound care, and patient assessments. The position requires participation in Humana's TB screening program and driver safety program.
Requirements
- Med-surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver's license, auto insurance, and reliable transportation.
Nice To Haves
- One year of nursing experience.

Responsibilities
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients' conditions and report changes.
- Educate patients and their families on disease management, medication, and treatment options.
- Maintain accurate records of patient care and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient's physician and Clinical Manager.
- Oversee the work of Home Health Aides and monitor their appropriate completion of documentation.
