About The Position
As a CenterWell Home Health nurse, reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence in their homes. You work with physicians and other clinicians to manage care plans that support recovery. The role is patient-facing and involves travel to patients’ homes using reliable transportation. This is a per-diem position with one scheduled weekly hour. Compensation is listed at a pay-per-visit/unit rate of $49.00–$69.00. The posting also lists a full-time base-pay estimate of $77,200–$106,200 per year, which may vary by location and job-related qualifications. Humana requires TB screening for this role and participation in its driver safety program, including maintaining a valid state driver’s license and required personal vehicle liability insurance.
Requirements
- Med-surg, ICU, ER, or acute-care experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.
- Meet the applicable personal vehicle liability insurance requirements.
- Complete TB screening if selected for the role.

Responsibilities
- Provide skilled nursing care in patients’ homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and families about disease management, medication, and treatment options.
- Maintain accurate patient-care records and coordinate with other healthcare professionals.
- Report patient-care and condition progress to the patient’s physician and Clinical Manager.
- Oversee Home Health Aides and monitor the appropriate completion of their documentation.
