About The Position

As a CenterWell Home Health nurse reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence in their homes. You work with physicians and other clinicians to manage care plans that support recovery. The position is patient-facing and includes working in patients’ homes and complying with Humana’s TB screening and driver safety programs. Scheduled weekly hours are 40. The posting gives a pay-per-visit/unit rate of $54.00–$75.00 and an estimated annual base pay range of $84,900–$116,800 for full-time employment; actual compensation may vary by location and job-related factors.

Requirements

• Med-surg, ICU, ER, or acute-care experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver’s license, auto insurance, and reliable transportation. The role requires maintaining personal vehicle liability insurance at the applicable state minimum or the stated coverage limits, whichever is higher.
• Must complete TB screening if selected for the role.

Responsibilities

• Provide skilled nursing care in patients’ homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients’ conditions and report changes.
• Educate patients and families on disease management, medication, and treatment options.
• Maintain accurate patient-care records and coordinate with other healthcare professionals.
• Report patients’ care and condition progress to their physicians and the Clinical Manager.
• Oversee Home Health Aides and monitor their completion of documentation.
