Home Health Registered Nurse
About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence in their homes. Working with physicians and other clinicians, you manage care plans that support recovery. The role involves providing skilled nursing care in patients’ homes, including medication administration, wound care, and patient assessments. You develop and implement individualized care plans, monitor patient conditions, educate patients and families, maintain care records, coordinate with healthcare professionals, and oversee Home Health Aides. The position includes patient-facing work and requires driving to patients’ homes.
Requirements
- Med surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and their families on disease management, medication, and treatment options.
- Maintain accurate records of patient care and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient’s physician and Clinical Manager.
- Oversee the work of Home Health Aides and monitor their appropriate completion of documentation.
