About The Position

Providence at Home, a division of Providence Life Services, provides senior-focused healthcare and home-based services. The Home Health Registered Nurse serves patients in the Palos Heights, Illinois area within a 25-mile radius. The role includes assessing patients and families to determine home care needs, developing and updating individualized plans of care, and carrying out preventive and rehabilitative nursing procedures. The schedule is Monday through Friday during normal business hours. On-call duties receive additional compensation, and mileage is reimbursed at $0.76 per mile.

Requirements

• Must be a Licensed Registered Nurse (RN) in the State of Illinois.
• Minimum of 1–2 years of experience, including at least one year in home care nursing, hospice, emergency room, or medical-surgical nursing.
• Must be CPR certified.

Responsibilities

• Complete initial assessments of patients and families to determine home care needs, including physical assessments and histories of current and previous illnesses.
• Regularly reevaluate patients’ nursing needs.
• Initiate and revise plans of care as patient status and needs change.
• Use health assessment data to determine nursing diagnoses.
• Develop care plans with goals based on nursing diagnoses and therapeutic, preventive, and rehabilitative nursing actions, involving patients and families in the planning process.
• Initiate appropriate preventive and rehabilitative nursing procedures.
