About The Position

Home Care Providers of Texas is hiring full-time registered nurses in the San Antonio, Texas area to provide skilled nursing care in patients’ homes. Nurses work in accordance with physicians’ orders and individualized plans of care, and collaborate with an interdisciplinary team to develop, implement, and update personalized care plans. The role includes administering nursing treatments, managing clinical activities, assessing patient progress and service effectiveness, and communicating findings with patients, families, physicians, case managers, and the care team.

The team uses the Roger app, described as an AI-driven, voice-enabled, HIPAA-compliant documentation platform, to streamline documentation and support OASIS requirements. Electronic charting is also provided through Kinnser (WellSky). The posting describes a remote workforce, with more than 90% of time spent on patient care, and offers clinical and administrative support around the clock.

Requirements

• Active Texas or compact registered nurse license.
• Active CPR certificate.
• Prior experience in geriatric home health care.
• Organizational skills and ability to work independently.
• Compassionate approach to care.
• Valid driver’s license.

Responsibilities

• Provide high-quality, comprehensive nursing care in patients’ homes in accordance with physicians’ orders and individualized plans of care.
• Partner with the interdisciplinary care team to develop, implement, and update personalized care plans.
• Administer nursing treatments and manage clinical activities to support patient health outcomes.
• Assess patient progress and the effectiveness of services, and communicate findings with the care team, patient, family, physician, and case manager.
• Perform additional duties as needed.
