About The Position
Home Care Providers of Texas is seeking compassionate, detail-oriented full-time Registered Nurses in the San Antonio, Texas area. The role provides skilled nursing care in patients’ homes and involves working with an interdisciplinary care team to support individualized patient outcomes. The team uses Roger, an AI-driven, voice-enabled, HIPAA-compliant documentation platform intended to reduce charting time while helping nurses stay current with OASIS requirements. Electronic charting is also provided through the Kinnser (WellSky) system. The position offers flexibility and support, including 24/7 clinical and administrative staff assistance and paid training and onboarding.
Requirements
- Active Texas or Compact RN license.
- Active CPR certificate.
- Prior experience in geriatric home health care.
- Self-starter with organizational skills.
- Compassionate approach to care.
- Valid driver’s license.
Responsibilities
- Provide comprehensive skilled nursing care in patients’ homes in accordance with physician orders and individualized plans of care.
- Collaborate with the interdisciplinary care team to develop, implement, and update personalized care plans.
- Administer nursing treatments and manage clinical activities to support optimal health outcomes.
- Assess patient progress and service effectiveness, and communicate findings with the care team, patient, family, physician, and case manager.
- Perform additional duties as needed.
