About The Position

Dependable Home Health of Tucson is seeking a compassionate and dedicated Registered Nurse (RN) to provide home health care to patients primarily within the 85706 and 85756 zip codes. This is a great opportunity for an RN who wants a more focused geographic territory, meaningful one-on-one patient care, and the independence of working in home health while still having the support of an experienced clinical team. Requirements

Current Arizona RN license in good standing
Minimum of one year of RN experience preferred
Current CPR certification
Valid driver’s license and reliable transportation
Strong clinical assessment and critical-thinking skills
Excellent communication, organization, and time-management skills
Ability to work independently while collaborating with an interdisciplinary team
Commitment to compassionate, patient-centered care

Nice To Haves

Home health, hospice, rehabilitation, med-surg, or critical care experience is a plus

Responsibilities

Conduct comprehensive in-home patient assessments
Develop and update individualized plans of care
Administer medications, treatments, injections, and wound care according to physician orders
Monitor patient progress and recognize changes in condition
Communicate patient updates and concerns with physicians and the interdisciplinary care team
Educate patients and caregivers on medications, disease management, safety, and wellness
Coordinate care with therapists, social workers, physicians, and other healthcare professionals
Supervise and document care provided by LPNs and Home Health Aides (HHAs), as appropriate
Maintain timely, accurate, and compliant clinical documentation
Help patients maintain their safety, independence, and quality of life at home
