Home Health Registered Nurse Case Manager
About The Position
As a CenterWell Home Health nurse, reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence in their homes. Working with physicians and other clinicians, you manage care plans that support recovery. The role involves providing skilled nursing care in patients’ homes, including medication administration, wound care, and patient assessments. You develop and implement individualized care plans, monitor patient conditions, educate patients and families, maintain accurate care records, coordinate with healthcare professionals, report progress to physicians and the Clinical Manager, and oversee Home Health Aides’ work and documentation. This is a patient-facing position subject to Humana’s TB screening program. It is also part of Humana’s driver safety program and requires a valid state driver’s license, personal vehicle liability insurance meeting applicable minimums, and reliable transportation. Scheduled weekly hours are 40. Compensation is listed as $49.00–$69.00 per visit/unit, with an estimated annual base pay range of $77,200–$106,200 for full-time employment. Pay may vary by geography and job-related skills, knowledge, experience, education, and certifications.
Requirements
- Med-surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and their families on disease management, medication, and treatment options.
- Maintain accurate records of patient care and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient’s physician and Clinical Manager.
- Oversee the work of Home Health Aides and monitor their appropriate completion of documentation.
