About The Position

Acts is currently seeking qualified candidates for our Home Health Physical Therapist position. This role is part of an in-house program within a welcoming, slower-paced community environment that delivers holistic care with a strong focus on prevention. The position offers specialized on-site training delivered during paid work hours, with some sessions eligible for CEUs. It also features state-of-the-art rehab gyms and equipment. There is an opportunity to build relationships by following residents across the full continuum of care, including inpatient, home health, outpatient, and fitness services. Clinically driven, patient-centered treatment decisions guide all programs and care plans. Cross training is provided to support all rehabilitation areas within our communities. There is limited travel within and between our private communities. Due to our company being a non-profit organization, student loan forgiveness is available after 10 years! In this role, you will be responsible for providing physical therapy services to home health residents/clients in their home environment to promote functional independence, safety, and quality of life. You will evaluate residents/clients, develop individualized plans of care, implement evidence-based interventions, and collaborate with the interdisciplinary care team to achieve optimal outcomes in compliance with physician orders, state practice acts, and federal and state home health regulations.

Requirements

  • Master’s degree or Doctoral degree in Physical Therapy from an accredited program
  • Valid Physical Therapist license or eligibility for license in the state(s) where services are rendered
  • Compliance with Florida's Care Provider Background Screening process

Nice To Haves

  • Minimum of 1-2 years of experience in home health is preferred
