About The Position

Acts is currently seeking qualified candidates for our Home Health Physical Therapist position. In this role, you will be responsible for providing physical therapy services to home health residents/clients in their home environment to promote functional independence, safety, and quality of life. Evaluates residents/clients, develops individualized plans of care, implements evidence-based interventions, and collaborates with the interdisciplinary care team to achieve optimal outcomes in compliance with physician orders, state practice acts, and federal and state home health regulations. This is an in-house program within a welcoming, slower-paced community environment that delivers holistic care with a strong focus on prevention. Specialized on-site training is delivered during paid work hours, with some sessions eligible for CEUs. The role offers state-of-the-art rehab gyms and equipment, and the opportunity to build relationships by following residents across the full continuum of care, including inpatient, home health, outpatient, and fitness services. Clinically driven, patient-centered treatment decisions guide all programs and care plans. Cross training is provided to support all rehabilitation areas within our communities. There is limited travel within and between our private communities. Due to our company being a non-profit organization, student loan forgiveness is available after 10 years!

Requirements

  • Master’s degree or Doctoral degree in Physical Therapy from an accredited program
  • Valid Physical Therapist license or eligibility for license in the state(s) where services are rendered
  • Compliance with Florida's Care Provider Background Screening process

Nice To Haves

  • Minimum of 1-2 years of experience in home health is preferred
