About The Position

A small, nurse-owned home health agency is seeking a Physical Therapist (PT) to provide care to patients in their homes. This role offers exceptional flexibility, allowing the PT to create their own schedule and determine their workload, from per-diem visits to a full-time caseload. The agency provides a supportive, relaxed work environment within a close-knit team, without the complexities of a large corporation. The primary service area includes Milton, Quincy, Randolph, Canton, and Dorchester, with potential cases in Weymouth, Braintree, and Roxbury.

Requirements

• PT Degree (Bachelor’s, Master’s, or Doctorate)
• Active PT license in MA
• At least 1 year of treating PT experience
• Reliable transportation for local patient visits
• Ability to work independently in patients’ homes
• Authorized to work in the United States; employer-sponsored work visas are not available for this position

Nice To Haves

• No initial OASIS visits—the RN owner completes them
• Work alongside an experienced and highly regarded PTA

Responsibilities

• Evaluate and treat patients in their homes
• Develop individualized treatment and rehabilitation plans
• Help patients improve their strength, mobility, balance, and independence
• Collaborate with the PTA, nurses, physicians, and other members of the care team
• Provide therapeutic interventions and home exercise recommendations
• Assist with care coordination and discharge planning
• Complete required clinical documentation
