About The Position
Concierge Home Care is seeking a Home Health Physical Therapist (PT) who desires more control over their schedule, meaningful patient relationships, and the ability to increase income based on productivity. This role offers a flexible schedule and autonomy, allowing for 1:1 patient care in the home. The coverage area is Sumter County, FL 32159, with territories designed to minimize drive time through clustered scheduling.
Requirements
- Active, unencumbered Florida Physical Therapist license
- Valid driver’s license, reliable transportation, and auto insurance
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
- Complete documentation using streamlined technology
