About The Position

Concierge Home Care is seeking a Home Health Physical Therapist (PT) to join their team. This role offers greater control over your schedule, the opportunity for meaningful patient relationships, and the ability to increase income based on productivity. The position is located in the Bradenton / Manatee County, FL area, with territories designed to minimize drive time through clustered scheduling. The company utilizes AI-assisted charting and a user-friendly EMR (WellSky/Kinnser) to reduce documentation time, allowing therapists to focus more on patient care.

Requirements

• Active, unencumbered Florida Physical Therapist license
• Valid driver’s license, reliable transportation, and auto insurance
• Employment is contingent upon successful completion of an AHCA Level 2 background screening.

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on safety and recovery
• Collaborate with physicians and interdisciplinary teams
• Supervise Physical Therapist Assistants (PTAs)
• Complete documentation using streamlined technology
