About The Position

As a therapist at CenterWell Home Health, the Home Health Physical Therapist Assistant reports to the Clinical Manager and helps patients regain strength, mobility, and independence in their homes. The role delivers personalized care focused on functional improvement and administers the physical therapy treatment plan developed by the Physical Therapist. Care is provided onsite in patients’ homes.

Requirements

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population.
Read, write, and speak fluent English.

One year of experience as a Physical Therapy Assistant.

Responsibilities

Administer the Physical Therapist-developed treatment plan to patients in their homes.
Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and heat, light, cold, water, and electrical modalities.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in-services.
Notify the Physical Therapist or Branch Director about equipment failures, supply shortages, or changes in staff or clients.
Apply infection-control concepts and universal precautions while performing patient care activities.
