About The Position

Sierra Home Health is seeking a Physical Therapist Assistant (PTA) to join its compassionate, collaborative home health team. The role provides in-home physical therapy services under the supervision of a licensed Physical Therapist, helping patients regain mobility, function, and independence. The position offers flexible scheduling, autonomy, supportive leadership, and the opportunity to serve the local community. The PTA will travel to Truth or Consequences one to two days a week, with mileage compensation. Occasional travel to T or C is preferred. Requirements

Current licensure as a Physical Therapist Assistant in New Mexico.
Strong interpersonal and communication skills.
Confidence working independently in the field.
Adaptability to changing schedules and home environments.
Commitment to quality, safety, and patient-centered care.
Reliable transportation and a valid driver's license.

Nice To Haves

Previous home health experience is preferred but not required.
Occasional travel to T or C is preferred.

Responsibilities

Deliver in-home physical therapy services under the supervision of a licensed Physical Therapist.
Carry out treatment plans to help patients regain mobility, function, and independence.
Communicate patient progress and changes in condition to the supervising therapist.
Educate patients and caregivers on exercises and safety practices.
Document visits and progress notes accurately and on time.
Participate in care team meetings and interdisciplinary planning.
Uphold infection control and safety standards during home visits.
Maintain professional, compassionate rapport with patients and families.
Travel to Truth or Consequences one to two days a week.
