About The Position

CenterWell Home Health is hiring a Home Health Physical Therapist Assistant to help patients regain strength, mobility, and independence in their homes. Reporting to the Clinical Manager, the assistant administers the treatment plan developed by a Physical Therapist and provides personalized care focused on functional improvement and quality of life. This patient-facing role includes home visits and requires participation in Humana's TB screening and driver safety programs. The position is scheduled for 40 hours per week. The posting lists a pay-per-visit/unit rate of $41.00–$57.00 and a full-time annual base pay range of $64,000–$87,500; compensation may vary by location and job-related qualifications.

Requirements

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population.
Fluent English reading, writing, and speaking skills.

Responsibilities

Administer the physical therapy treatment plan developed by the Physical Therapist to patients in their homes.
Provide non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and heat, light, cold, water, and electrical modalities.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in-services.
Notify the Physical Therapist or Branch Director of equipment failures, supply shortages, or changes involving staff or clients.
Apply infection-control concepts and universal precautions while performing patient care activities.
