About The Position
StellarCare Staffing is seeking a PRN Occupational Therapist (W2) to join their team. This role involves providing high-quality home health services to patients, helping them regain independence in their daily activities. The therapist has the flexibility to choose coverage areas and workdays, while StellarCare handles other aspects of employment. The position emphasizes elite clinical support and work-life balance.
Requirements
- A degree in Occupational Therapy from an accredited program.
- NBCOT certification.
- A valid state license.
- Minimum of 1 year of clinical experience.
- A valid driver’s license, auto insurance, and dependable transportation.
- Comfort with electronic documentation and mobile communication.
Nice To Haves
- Home Health experience preferred.
Responsibilities
- Conduct initial evaluations and develop goal-oriented plans focused on ADLs (Activities of Daily Living) and IADLs.
- Provide one-on-one therapeutic interventions, including fine motor coordination, cognitive retraining, adaptive equipment training, and activities within your scope of practice.
- Perform comprehensive home safety assessments and recommend modifications or specialized equipment to foster aging-in-place.
- Complete timely OASIS assessments and progress notes via our intuitive EMR system.
- Work closely with physicians and the StellarCare clinical team to ensure seamless patient transitions and holistic care.
