About The Position

Beacon Home Health is seeking an Occupational Therapist to serve patients in Smithville, Tennessee, and surrounding communities. This PRN role assesses and evaluates patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs associated with medical conditions. The therapist provides care within the scope of state practice laws, follows the plan of care, uses evidence-based techniques, and coordinates with other members of the healthcare team.

The position requires travel to multiple patient residences throughout the assigned geographic area. Requirements

• Current Occupational Therapist license in the state where services are rendered.
• Current CPR certification.
• Valid driver’s license and vehicle insurance.
• Access to a dependable vehicle or public transportation.
• Ability to reliably commute each day to multiple patient residences within the assigned geographic area.

Responsibilities

• Provide occupational therapy services within the scope of practice defined by applicable state laws, in accordance with the plan of care, using evidence-based techniques, and coordinating with other healthcare team members.
• Evaluate patients’ functional status, body systems as required for CMS documentation, and occupational therapy needs.
• Consult with the physician in developing the therapy plan of care.
• Observe, record, and report patients’ responses to treatment and changes in condition to the nurse supervisor and physician.
• Assess patients’ level of function using diagnostic and prognostic functional ability tests.
