About The Position

BAYADA Home Health Care is seeking an experienced Occupational Therapist for a part-time role providing home health visits to adult and geriatric patients in their homes in Westford, Groton, Littleton, Acton, and surrounding towns. Prior home health experience is not required. The Occupational Therapist is an integral member of a multidisciplinary health care team providing rehabilitative care and skilled nursing so clients can receive the care they need to remain at home. The position offers a flexible schedule and work/life balance. Compensation is $62.00–$68.00 per point, depending on qualifications. Requirements

Current Massachusetts Occupational Therapist license.
At least one year of recent work experience as an Occupational Therapist.

Responsibilities

Make home visits to clients in designated geographic territories.
Evaluate patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans.
Evaluate home environments for hazards or barriers to independent living.
Identify equipment needs and environmental adaptations to improve client outcomes.
Continually assess and revise occupational therapy care plans, and participate with nursing in multidisciplinary care planning as appropriate.
Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities that promote health, safety, and independent living.
Document observations, interventions, evaluations, and services provided using a touch pad tablet.
