About The Position

ACE Home Health Care & Hospice is seeking an experienced, field-based Home Health Occupational Therapist to serve patients in San Jose, California. The role offers autonomy and flexibility in managing hours within the assigned territory, with support from an in-house team. Clinicians have access to AI-powered charting tools, dedicated administrative coordinators, durable medical equipment support, and individualized, structured, ongoing professional development and training. Compensation is based on a per-visit pay structure, with estimated full-time annual compensation of $153,000–$206,000. Mileage is reimbursed at the federal rate, and paid parking is provided. Requirements

At least 1 year of home health experience.
Graduate of an accredited Occupational Therapy program.
Registered with the National Registration Examination of the American Occupational Therapy Association.
At least 2 years of clinical occupational therapy experience.
Strong verbal, written, and organizational skills.
Current CPR certification.
Valid driver's license.
