About The Position
Activa Home Health, an AccordCare Company, is seeking a Field Registered Nurse to join their team in Boynton Beach, FL. They value clinical excellence and are looking for individuals to perform occupational therapy evaluations and reassessments in the home setting. The role involves developing and implementing individualized treatment plans, providing therapeutic interventions to improve ADLs, mobility, strength, coordination, and safety, and educating patients and caregivers. The position also requires timely and accurate documentation and collaboration with the interdisciplinary care team, while maintaining compliance with Medicare and agency guidelines.
Requirements
- Current and active Florida Occupational Therapist (OT) license
- Minimum of one (1) year of Home Health experience preferred
- Medicare Home Health experience strongly preferred
- Experience with EMR systems (WellSky/Kinnser preferred)
- Strong clinical assessment and communication skills
- Current CPR certification
- Valid driver’s license and reliable transportation
- This role requires a screening through the Care Provider Background Screening Clearinghouse.

Responsibilities
- Perform occupational therapy evaluations and reassessments in the home setting
- Develop and implement individualized treatment plans based on patient needs and physician orders
- Provide therapeutic interventions focused on improving activities of daily living (ADLs), mobility, strength, coordination, and safety
- Educate patients, caregivers, and family members on treatment goals and home exercise programs
- Complete all required documentation accurately and timely
- Collaborate with physicians, nurses, PTs, and interdisciplinary care team members
- Maintain compliance with Medicare and agency guidelines
