About The Position

Matrix Home Care is currently seeking a compassionate, dependable, and experienced bilingual Certified Nursing Assistant (CNA) or Home Health Aide (HHA) to provide one-on-one care to clients in their homes throughout Boynton Beach and surrounding areas. This opportunity is ideal for a caregiver who enjoys working directly with clients, helping them remain safe and independent in their homes, and building meaningful relationships with patients and families. We are looking for someone who is professional, reliable, patient, and comfortable working independently in the home setting.

Requirements

  • Current Certified Nursing Assistant (CNA) certification or Home Health Aide (HHA) certificate
  • Ability to communicate in English and Spanish
  • Current CPR or BLS certification
  • Current health statement or physical, as required
  • Current TB screening or other required health documentation
  • HIV/AIDS training certificate
  • Alzheimer's Disease and Related Disorders training, if applicable
  • Medication Assistance training, if applicable
  • Medical Errors training, if applicable
  • Level 2 AHCA background screening or ability to complete required screening
  • Professional liability insurance, if required
  • Valid driver's license
  • Current automobile insurance if driving or traveling between cases
  • Reliable transportation
  • Ability to work independently and follow a client's care plan
  • Strong communication and documentation skills
  • Dependable attendance and punctuality

Nice To Haves

  • Previous experience in home health, private duty, assisted living, skilled nursing, senior care, or a related healthcare setting preferred

Responsibilities

  • Assist with bathing, showering, grooming, and personal hygiene
  • Assist clients with dressing and changing clothing
  • Provide toileting and incontinence care as needed
  • Assist with transferring, positioning, walking, and mobility
  • Provide fall-prevention and safety assistance
  • Assist with feeding and meal preparation
  • Prepare simple meals based on the client's dietary needs
  • Provide medication reminders within the caregiver's permitted scope
  • Assist with light housekeeping related to the client's care
  • Change bed linens and maintain a clean client environment
  • Provide companionship and emotional support
  • Accompany or assist clients with activities when required by the care plan
  • Observe the client's physical and emotional condition
  • Report changes or concerns to the appropriate clinical or office staff
  • Maintain accurate documentation of services provided
  • Follow the established plan of care and agency policies
  • Maintain client confidentiality and comply with HIPAA requirements
  • Communicate professionally with clients, family members, nurses, and office staff
