Home Health Nurse- LPN/ RN
About The Position
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting. The client is a 63-year-old man who is in hospice and bed-bound. He lives in a clean, smoke-free home with his wife and 3 cats. His medical status puts him at high risk for an adrenal crisis, particularly from stressors like infection. His wife is supportive and easy to work with. The home is neat and clean. The client enjoys sports, watching TV, and playing games.
Requirements
- Active RN or LPN license in the state of Michigan
- Strong communication and critical thinking skills
Nice To Haves
- Home care or private duty experience
- Trach/vent experience
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care)
- Monitor and document patient condition and vital signs
- Communicate with families and care teams
- Maintain safe, high-quality, patient-centered care
- Tube feeding
- Manage hypopituitarism and multiple complex comorbidities
- Monitor for and manage risk of adrenal crisis
