Home Health Nurse- LPN/ RN
About The Position
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting. This specific role involves caring for a 63-year-old male patient in hospice care who is bed-bound. He lives in a clean, smoke-free home with his wife and 3 cats in Mt Pleasant. His care includes tube feeding, managing hypopituitarism and multiple complex comorbidities, and monitoring for high risk of adrenal crisis, especially from stressors like infection. The patient enjoys watching TV and playing games. The client's wife is supportive and easy to work with, fostering a respectful professional atmosphere.
Requirements
- Active RN or LPN license in the state of Michigan
- Strong communication and critical thinking skills
Nice To Haves
- Home care or private duty experience preferred
- Trach/vent experience a plus (training provided)
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care)
- Monitor and document patient condition and vital signs
- Communicate with families and care teams
- Maintain safe, high-quality, patient-centered care
- Tube feeding
- Manage hypopituitarism and multiple complex comorbidities
- Monitor for high risk of adrenal crisis, particularly from stressors like infection
