Requirements

Graduate of an accredited Nursing Program with an active RN license in IL
Minimum 2 years of nursing experience, ideally in home healthcare
Current CPR and BLS certifications
Knowledge of state and federal home health regulations
Strong clinical judgment, excellent communication skills, and a compassionate approach

Responsibilities

Assess the patient's and family caregiver's homecare needs and coordinates home health care services as required prior to the patient's transition home
Acts as clinical resource to coordinate complex cases for safe and appropriate transition to the patient's home
Examine patients, conduct assessments, and document patient progress while effectively communicating with other healthcare providers
Educate patients and their families on care plans, medications, therapies, diet, and exercise
Complete and present patient reports to facilities and management teams
Works collaboratively with Office Intake Department in verification of coverage or payment
