About The Position
Elara Caring is seeking a compassionate Medical Social Worker (MSW) to provide home health social work services. The role assesses patients’ psychosocial, financial, environmental, and safety needs; develops and updates individualized care plans; and coordinates with patients, families, physicians, and the care team to support continuity of care and positive outcomes. The position is PRN.
Requirements
- Current state license as a Social Worker.
- Master’s or doctoral degree in Social Work from a school accredited by the Council on Social Work Education.
- At least 1 year of social work experience in a healthcare setting.
- Social work skills as defined by general social work standards.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver’s license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
Responsibilities
- Assess patients to identify psychosocial, financial, environmental, and safety needs.
- Complete initial social work evaluations and reassess patient needs during follow-up visits.
- Develop and update individualized care plans based on patient needs, goals, and physician orders.
- Communicate changes in patient condition, concerns, or safety issues to physicians and the care team.
- Connect patients and families with appropriate community resources and support services.
- Counsel and educate patients and families to help address social, emotional, and care-related needs.
- Coordinate with the care team to support continuity of care and effective discharge planning.
- Provide compassionate, high-quality social work services that support positive patient outcomes.
