About The Position

Elara Caring is seeking a compassionate Medical Social Worker (MSW) to provide home health social work services. The role supports patients by assessing psychosocial, financial, environmental, and safety needs; completing evaluations and follow-up reassessments; and developing and updating individualized care plans based on patient needs, goals, and physician orders. The social worker communicates changes and concerns to physicians and the care team, connects patients and families with community resources, provides counseling and education, and supports continuity of care and discharge planning. The position requires travel between patient homes and assigned work locations.

Requirements

• Current state license as a Social Worker.
• Master’s degree or doctoral degree in Social Work from a school of Social Work accredited by the Council of Social Work Education.
• At least 1 year of social work experience in a healthcare setting.
• Social work skills as defined by general social work standards.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver’s license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Assess patients to identify psychosocial, financial, environmental, and safety needs.
• Complete initial social work evaluations and reassess patient needs during follow-up visits.
• Develop and update individualized care plans based on patient needs, goals, and physician orders.
• Communicate changes in patient condition, concerns, or safety issues to physicians and the care team.
• Connect patients and families with appropriate community resources and support services.
• Counsel and educate patients and families to address social, emotional, and care-related needs.
• Coordinate with the care team to support continuity of care and effective discharge planning.
• Provide compassionate, high-quality social work services that support positive patient outcomes.
