About The Position

WelbeHealth PACE helps seniors remain in their homes and communities through comprehensive medical care and community-based services. The Home Health Manager manages skilled home healthcare and establishes the framework that enables the home health team to support participants. The role is accountable for delivering outstanding care in participants’ homes and overseeing care in nursing homes and board-and-care homes, including arranging and supervising nursing, physical therapy, and occupational therapy.

Requirements

• California Registered Nurse (RN) license required.
• Knowledge of nursing needed to treat frail, elderly participants and address intricate clinical scenarios.

Nice To Haves

• Bachelor’s degree preferred; additional years of relevant experience may be substituted.
• At least two years of nursing experience in a home health setting with a frail or elderly population preferred.

Responsibilities

• Direct a high-performing home care department and ensure workflows and policies are followed.
• Guide and maintain home care staff, including hiring, training, and coaching team members.
• Assess the effectiveness of home health nursing assessments and interdisciplinary team (IDT) participation.
• Ensure home care nurses deliver and document nursing interventions as agreed upon in participants’ care plans.
• Oversee the quality of care provided to participants in nursing homes and board-and-care facilities.
