About The Position

The Home Health Licensed Vocational Nurse — LVN provides skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse and in accordance with the patient’s individualized plan of care. The LVN plays an important role in supporting patient safety, promoting independence, monitoring changes in condition, and communicating effectively with the clinical team. The LVN is responsible for providing compassionate, patient-centered care while following agency policies, physician orders, regulatory requirements, and professional standards of practice.

Requirements

  • Current and active California Licensed Vocational Nurse license.
  • Current CPR/BLS certification.
  • Ability to work independently in the field while following the direction of the supervising RN.
  • Strong clinical observation, communication, and documentation skills.
  • Comfortable providing care in a variety of home environments.
  • Reliable transportation, valid driver’s license, and current auto insurance.
  • Ability to use electronic medical record systems and mobile documentation tools.
  • Demonstrates compassion, professionalism, dependability, and good judgment.

Nice To Haves

  • Minimum of one year of nursing experience preferred.
  • Home health experience preferred but not required.

Responsibilities

  • Provide skilled nursing visits to home health patients as assigned and within LVN scope of practice.
  • Follow the established plan of care and report any patient changes, concerns, or needs to the supervising RN or Clinical Manager.
  • Perform treatments and procedures as ordered, including wound care, medication support, vital signs, disease process monitoring, and patient/caregiver education.
  • Reinforce teaching provided by the RN, therapist, or provider regarding medications, safety, infection control, disease management, and the home care plan.
  • Observe and document the patient’s condition, response to care, and progress toward goals.
  • Communicate timely and accurately with the interdisciplinary team regarding changes in condition, missed visits, patient concerns, or care coordination needs.
  • Maintain accurate, complete, and timely clinical documentation in accordance with agency standards and regulatory requirements.
  • Promote patient safety in the home setting, including fall prevention, infection prevention, medication safety, and emergency preparedness.
  • Support continuity of care by coordinating with patients, caregivers, physicians, pharmacies, and other care providers as appropriate.
  • Participate in case conferences, team communication, and quality improvement efforts as requested.
  • Maintain patient confidentiality and comply with HIPAA and all applicable laws and regulations.
  • Represent the agency professionally in patient homes and community settings.
