StellarCare Staffing is seeking a PRN Licensed Vocational Nurse (LVN) to join our team. The Role As a PRN LVN with StellarCare, you will provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse, helping them maintain health, comfort, and independence. You pick the coverage areas and the days you want to work—we handle the rest.
Requirements
- Graduate of an accredited vocational nursing program with a current, unrestricted Licensed Vocational Nurse (LVN) license in the state of practice.
- Minimum of 1 year of nursing experience required.
- Valid driver's license, current auto insurance, and dependable transportation.
- Comfort with electronic documentation, mobile communication, and EMR systems.
- Strong patient care, communication, and observation skills with the ability to work independently while following RN supervision requirements.
- Ability to practice within LVN scope of practice and agency policies, as well as state and federal regulations governing home health services.

- Home Health experience preferred.
Responsibilities
- Provide Skilled Nursing Care: Deliver nursing services in accordance with the patient’s plan of care, including medication administration, wound care, vital sign monitoring, and disease management support.
- Support Patient Independence: Reinforce education provided by the RN and assist patients and caregivers in managing chronic conditions and daily healthcare needs.
- Monitor Patient Status: Observe, document, and promptly report changes in patient condition to the supervising RN and physician as appropriate.
- Documentation: Complete timely and accurate visit notes and required clinical documentation through our EMR system.
- Collaborate: Work closely with RNs, physicians, therapists, and the StellarCare clinical team to ensure coordinated, patient-centered care.
