Requirements
- Active LPN license in the state of South Carolina
- Prior home health, long-term care, or clinical nursing experience preferred
- Strong knowledge of medication administration, wound care, and patient assessment
- Excellent communication and interpersonal skills with patients and families
- Reliable transportation and a valid driver's license for home visit travel
- Ability to work independently and manage time effectively in a field-based role
- Compassionate, patient-focused demeanor with a commitment to quality care
Responsibilities
- Monitor and document patient health status, vital signs, and progress toward care goals
- Communicate effectively with patients, families, and the interdisciplinary care team
- Educate patients and caregivers on disease management, medications, and healthy practices
- Maintain accurate and timely clinical documentation in compliance with home health regulations
- Identify changes in patient condition and report to supervising RN or physician promptly
